Dear Parent,

The forms listed here are for families using curriculum other than that provided by the Academy. All curriculum must be approved by the Academy and the forms below must be printed out and used for each course your child completes. These forms must be submitted each quarter and will remain in the Bridgeway - Home School Academy office. 

We extend our services to you as you undertake the very important task of schooling your children. We are available for assistance Monday through Friday from 10 a.m. to 4 p.m. We will do our best to help you succeed with your homeschooling experience.
